Player List Appeal Form - Manual Submission

Notice: Before you begin to fill this information out, be aware that we expect you to be as truthful as possible in answering all items on this form. The associations have rosters and lineup cards from all of their tournaments from the 2007 season and in many cases have record of MVP and all-tournament selections as well. Such references may be used to verify information that you present in your appeal.

Any gross misrepresentations of the facts on your part can and will be held against you and may be considered immediate grounds for denying your appeal.

This form may at first seem lengthy to you, but, in truth,  it should not take you more than five minutes to complete.  It includes the information necessary to properly evaluate the merits of your appeal.  So be careful to complete each question as applicable.
